The form is filled out here on the North Central Area
MARS website and will be sent by email to the appropriate staff person(s)
for processing. Once you have submitted the form, you will be contacted
by either your State MARS Director (SMD) or another Area or Region Official
to confirm the information provided and to obtain any additional materials
that may be required.

Click the link "Fill Out the Form" at
the end of these instructions. After a few moments, the form will
display on your screen. |

Using your mouse, point and click to the white box to the right
of "Name?". Enter your name. Use capitalization as appropriate. |

Use the TAB key to move to the successive areas
and enter the information requested. If you find it necessary to return
to a previous area, use you mouse and point to the area and then click
the left mouse button.
NOTES ON ENTERING DATA
DO NOT USE THE ENTER KEY or an incomplete form may
be submitted.
Use capitalization as appropriate.
When entering the State information select the Region
5 or Region 7 state that corresponds to your state of residence.
Comments are optional and you may enter as many as you
wish. |
